This form intends to connect high school students with Catholic chaplaincies in the Greater Toronto area.
Form completion is voluntary, as is providing contact information. Form submissions are accessed only by those who have completed diocesan Safe Environment screening processes, and are retained only for one year. If you choose to complete this form, a representative from the chaplaincy at your indicated institution will be in touch with you.
Please note that the Chaplaincy Connect website and associated database of collected personal information are hosted and maintained by the Office of Catholic Youth of the Archdiocese of Toronto using Google forms.
